Photo and Video Opt-Out Form

Completion and submission of this form permits you to 'opt out' having you and/or your child(ren)'s photos used on printed NSA materials, in video(s), or on the web and internet (eg: social media). At the start of any NSA event or gathering, you are considered to be 'opted in' (participating) unless you 'opt out' by using this form. You may opt out at any time. To opt out, this form must be completed and submitted to the NSA office.
